FOR TEACHERS, COUNSELORS AND SCHOOL STAFF
Are you a Teacher, Counselor or Advisor who would like to nominate a student who is highly motivated, driven and focused on pursuing their goal of attending college? Use our Scholar Nomination Form to tell us about them!
FOR PROSPECTIVE STUDENTS AND THEIR PARENTS/GUARDIANS
BEFORE YOU APPLY:
1. Please be sure you meet the following criteria:
- Attend (or will attend) one of the following high schools:
- Chatham Central High School
- Jordan-Matthews High School
- Lee County High School
(Students in the CCCC UB Program cannot be in both Programs) - Northwood High School
- Southern High School
- Hillside High School
- Durham School of Technology
- Maintain Citizenship, Permanent Residency, or are in the process of becoming a Permanent Resident of the United States
- Meet Federal Guidelines based on income and/or educational background of parents/guardians.
- Have at least a 2.5 GPA (4.0) or higher at time of application.
2. Gather the following materials to submit:
- A copy of your Parent/Guardians most recent Tax Return (Form 1040 or 1040A).
- Please note, a copy of your Parent/Guardian’s W-2 or Tax Transcript can not be accepted as proof of income.
- A copy of your Current Transcript.
- If you are a 9th grader and are applying before the end of your first semester, please submit a Report Card or Screenshot of current grades in PowerSchool.
- One Recommendation Form from a Teacher, School Counselor, or Community Member.
- If you are a 9th grader and do not have a Teacher or Counselor that can speak to your academic abilities, you may ask a Mentor, Advisor or other individual.

3. Submit an Application
- Complete Full Online Application
- Please review the criteria above before submitting an online application. Students must attend one of the listed high schools to be considered for the Program, no exceptions. At the end of the application, you will have the option to upload an forms mentioned above.
- Download the application
- If you would like to download a paper copy of the Application, please use the above link to do so. Please note, paper applications will need to be mailed, faxed or scanned to the CHEOP/Upward Bound Office.
After you have submitted your application, an Upward Bound Staff Member will reach out to you at the email address you provided to follow up regarding next steps. An interview is required for a Student to be admitted to the Program.
